Axial spondyloarthritis (axial SpA) is a chronic immune-mediated inflammatory condition of the axial skeleton associated with pain, stiffness, and progressive irreversible structural damage in the spine, leading to reduced physical function and health-related quality of life. In axial SpA, spinal structural damage is characterized by new bone growth like syndesmophyte formation and ankylosis of the axial skeleton. The major treatment goals in axial SpA are to maximize health-related quality of life through the control of symptoms, inflammation, and by avoiding radiographic spinal progression. Presented in this lecture is an up to date review of axial SpA management for improving outcomes of patients, focusing not only on clinical symptoms but also on the importance of inhibition of spinal radiographic progression.
